Individuals wishing to perform massage therapy must obtain a City Massage Therapist Permit. In addition to the application and fee, applicants must provide the following documents:

  • A copy of a valid government‑issued photo ID (proof of age)
  • A copy of a valid Missouri State Massage Therapist License
  • A completed City Massage Therapist Permit Application
  • If working in a commercial location, information about the licensed establishment where services will be performed.
