Mechanical, Electrical or Plumbing Contractor

We will need the following documents in addition to your application: 
  • A copy of your workers’ compensation insurance or the waiver of insurance.  
  • A copy of your general liability insurance 
    • Certificate of insurance evidencing a comprehensive general liability coverage policy with the minimum limits of five hundred thousand dollars ($500,000.00) for personal injuries and property damage  
    • Certificate Holder - City of Blue Springs 
  • A copy of your Master License and/or most recent continued education Test Score

Construction Related Contractors

We will need the following documents in addition to your application: 
  • A copy of your worker's compensation insurance or waiver

 Private Security

We will need the following documents in addition to your application: 
  • A completed Private Security Permit  
  • A copy of your general liability insurance 

Real Estate/Property Management/Leasing/ Landlord

We will only need a completed application.

Retail Business/Other Sales-Related Business

We will need the following documents in addition to your application: 
  • A copy of your Certificate of No Sales Tax Due – ONLY IF YOU SELL A PRODUCT

Nail Salon/Hair Salon/Massage Therapy

We will need the following documents in addition to your application: 
  • A copy of your drivers license 
  • A copy of your State License 
  • MASSAGE ONLY - Completed application of the massage permit. This can be provided to you. 
    • If a commercial location and you are the owner you will need to complete the Establishment License application. This can be provided to you. 
      • We will need a copy of the Massage Therapy Business from the state.
